Home and Community-Based Services (HCBS) are designed to help individuals live as independently as possible while receiving the support they need in their homes and communities. Our services are person-centered, flexible, and tailored to each individual’s goals, preferences, and assessed needs.

Basic Support Services

These services provide essential assistance with daily living and supervision to help individuals remain safe and supported.

  • 24-Hour Emergency Assistance: On-call support available at any time to respond to urgent needs, ensuring safety and peace of mind for individuals and their families.
  • Adult Companion Services: Non-medical supervision and social support to help individuals engage in community activities, build relationships, and reduce isolation.
  • Homemaker Services: Assistance with household tasks such as cleaning, laundry, meal preparation, and organizing the home to maintain a safe and healthy living environment.
  • Night Supervision: Overnight monitoring and support to ensure safety during sleeping hours, including assistance if needs arise during the night.
  • Respite Care (In-Home or Out-of-Home): Short-term care that gives primary caregivers a break while ensuring the individual continues to receive quality support in a safe environment.
  • Individual Community Living Support (ICLS): Personalized support that helps individuals live successfully in the community by building daily living skills, managing routines, and participating in community life.
  • Individualized Home Supports: Flexible support services delivered in the individual’s home or community, focused on developing independence and maintaining health and safety.

  • Individualized Home Supports with Training: Hands-on assistance with instruction for developing important life skills — such as personal care, budgeting, meal planning, and home-management tasks.
  • Individualized Home Supports with Family Training: Integrated support that educates and empowers family members or caregivers to help their loved one build lasting independence.
